Insurance Information

The Nephrology Clinic accepts payment from most insurance plans, including Medicare and Medicaid. General Insurance Information Insurance and Billing We are committed to providing you with the best possible care as well as establishing and maintaining a strong physician-patient relationship.  Helping you understand our financial policy before your visit allows a good flow of communication and enables us to achieve our goal.   Please verify with your insurance company that we are a participating provider in your network.  Please bring your insurance cards and any required referrals to all appointments. We ask that you provide this information at each visit to make sure that our records are correct, your policy is current, and that we participate with your plan.  As a courtesy to our patients we will submit claims for you.  Insurance coverage is an estimate; therefore patients are ultimately responsible to this office for all fees incurred. We will ask that you complete a Consent Release Form giving us the names of relatives, friends, or care-givers that we are allowed to talk to about your medical condition, as well as pick up prescriptions for you. According to your insurance plan, you are responsible for paying all copays, deductibles, and coinsurances at the time of your visit. Copays will be collected when you check in for your appointment. For your convenience we accept VISA, MasterCard, DISCOVER, AMERICAN EXPRESS, personal checks or cash. If you are seen at one of our out-reach clinics, we will mail you a self- addressed, stamped envelope so that you may send us your payment for the copay or coinsurance. Payment will be due within one week from appointment. Supplemental Insurance Coverage As a courtesy we will submit claims to your supplemental insurance company. If there is no response within 60 days, the balance will become your responsibility. No Insurance Coverage / Cash Services Non-covered services are collected at the time of service. A discount will be offered to uninsured patients. Collection Policy Billing statements will be sent to you, if necessary, after the insurance company processes the claim.  The balance on your account after insurance pays is due in 30 days.  The balance will be sent to an agency for collection if payment is not received within 110 days from date of service. We realize that temporary financial difficulties may affect timely payment of your account.  If such problems do arise, please contact our billing department.
